SUPPORT COORDINATOR JOB DESCRIPTION
The ESC Support Coordinator is an organized, motivated person, who works well in both independent and team settings. The Support Coordinator manages services for 35-40 individuals by performing the following four general functions: individual discovery, plan development, coordination of services, and monitoring. Your role will be to connect the individual with services and supports, maintain regular contact and complete required documentation.
Support Coordinator Responsibilities:
- Develop Individualized Service Plan (ISP) and Person-Centered Planning Tool (PCPT) for individuals enrolled in DDD/Medicaid services within 30 days of enrollment. This includes:
- Interview the individual/family; reviewing/compiling information obtained from various sources, such as service providers, assessments or evaluations.
- Assure the information in ISP and PCPT is understandable and written in a person-centered manner.
- Link the individual to service providers; assist in narrowing down the list of potential providers; reaching out to providers to confirm service capacity, determine intake/eligibility requirements, gather, and submit referral information as needed.
- Identify with the individual and service providers the ISP outcomes and confirm start date.
- Distribute the ISP (and PCPT when the individual consents) to the individual, all team members, and identified service providers; and review the ISP through monitoring conducted at specified intervals.
- Revise the ISP as needed – for example, when services are changed, reduced, or terminated.
- Develop a thorough understanding of the services funded by DDD and other state and community resources, to ensure that individuals/families are offered informed choice of services and providers.
- Ensure delivery of quality services and ensure that services are provided in a safe manner, in full consideration of the individual’s rights.
- Ensuring individuals served are free from abuse, neglect, and exploitation; reporting suspected abuse or neglect in accordance with specified procedures; and providing follow-up, as necessary. Ensuring that incidents are reported in a timely manner in accordance with policy and follow-up responsibilities are identified and completed.
- Notifying the individual regarding any pertinent expenditure issues.
- Conduct Monthly Monitoring
- Contacts monthly, face-to-face visits on a quarterly basis, and home visit on an annual basis that includes review of the ISP and documentation on the Support Coordinator Monitoring Tool.
- Complete Monthly Monitoring by the 25th of the month.
- Complete Monthly Monitoring Tool within 72 hours of contact.
- Indicate on ESC Caseload document when MMT is completed and ready for review by Supervisor or Quality Assurance Specialist.
- Maintain accurate case notes. Entering required information into the iRecord in an accurate and timely manner.
- Maintaining a confidential case record that includes, but is not limited to, the NJ Comprehensive Assessment Tool (NJ CAT), Support Coordinator Monitoring Tools, PCPTs, ISPs, notes/reports, annual satisfaction surveys, and other supporting documents uploaded to the iRecord.
- Complete at least 12 hours per year of continuing education training. This training will be in form of conferences and on-line webinars. Track training on ESC Professional Development Tracking Sheet.
Experienced Support Coordination Responsibilities
- Attend Monthly Staff Meetings.
- On-Call Rotation.
- Meet with Supervisor monthly, or more frequently as needed.
- Report data to the Division upon request.
- Submit expenses to CFO in timely manner.
- Submit time-off requests in accordance with ESC policies.
Required Knowledge/Skills/Attributes:
· Experience and proficiency in using Office 365 products – Word, Outlook, Microsoft Teams, Excel.
· Must have own transportation, a valid driver’s license, and insurance.
- Have strong and effective spoken and written communication skills.
- Respect and appreciation for people with disabilities, their families, other organizations, agencies, volunteers, and consumers.
- Committed to providing the highest level of customer service to ensure they are well informed and supported in the achievement of their goals.
- Able to work independently with minimal direction and reach out to ESC team when guidance is needed.
- Represent ESC in a professional demeanor and presentation.
Education and Experience:
- Bachelor’s Degree
- DDD Support Coordinator Training
- College of Direct Support Trainings
- One year experience working with adults with intellectual or developmental disabilities (experience can be paid, volunteer or personal)
- Fingerprinting, Federal and State background checks required.
Working Conditions:
- Standard Work Week is 9:00 AM – 5:00 PM with one hour lunch break (35 hours).
- On-site – 1 day per week in Hamilton, NJ office.
- Requires flexibility to meet the requests for consumer/families meeting after standard working hours.
- Requires in-home visits and attendance at community-based meetings, therefore Support Coordinator must have reliable transportation.
